Healthcare time ratio

The cost/benefit calculations for healthcare sometimes use QALY's (Quality adjusted life years). But since the cost can be measured in work-hours, let's measure the benefit in terms of hours of life gained as well (even if QALH's doesn't slip of the tongue). What ratio of time worked to time gained would you consider acceptable. Should it be significantly less than 1, or is it worthwhile to put in more time contributing to someone's health than they will get back as a result?
